This page is for information about purchasing a subscription from Portage Area Ambulance Association.

The subscription program is neither an insurance contract nor a solicitation of insurance premium.  The subscription program is designed to offset the cost of ambulance transportation. 

The SENIOR INDIVIDUAL PLAN applies only to the individual, who is 62 years of age or older.

The SINGLE INDIVIDUAL PLAN applies only to the individual, who is 26 years of age and older.

The FAMILY PLAN applies to husband, wife, and any children under 26 years of age or mentally or physically handicapped person living  at the residence. 

Until April 1, the subscription becomes effective upon receipt and acceptance of the subscription application by the Portage Area Ambulance Association.  The subscription expires on December 31.

  • Subscribers receive a 50% discount on any out-of-pocket expenses (e.g. deductibles, co-payments, etc.) related to medically necessary emergency ambulance service (provided by PAAA) to the closest appropriate medical facility.
  • Non-Subscribers will be billed the current rate for emergency and / or non-emergency transportation, must provide insurance information, and will be responsible for any balance.  All non-emergency transports must be medically necessary and have a Certification of Medical Necessity signed by an authorized person.
  • We reserve the right to determine necessity and we also reserve the right to determine eligibility.
  • No subscription will be required for people covered by PA Department of Welfare-ACCESS.

Average Insurance Reimbursement:$600.00
Balance due to Portage Area Ambulance Assocation:$400.00
Subscriber pays:$200.00 out of pocket
Non-Subscriber Pays:$400.00 out of pocket
